A 45 year old man presented with history of flash oedema and was admitted for accelerated hypertension at a cardiac centre.On evaluation; his work up was essentially normal except for a large cyst in right kidney.
The patient and the attendants took opinion from our doctors and he was subsequently evaluated for further anatomical and physiological details with CT scan abdomen and DTPA renogram.His CT scan showed a large simple cyst in right kidney becoming exophytic in lower pole and reaching the pelvic hilum.DTPA renogram showed decreased perfusion probably because of compression by the cyst on hilar vessels.

The case was discussed in details with the nephrologist and all the pros and cons regarding the cyst deroofing and its effect were discussed.As there was reduced perfusion because of hilar mass efefct and no other cause for hypertension discernible ; a decision for laparoscopic deroofing of the cyst was taken after counselling the patient.

The surgery was done uneventfully and the patient was discharged the second day.He is normotensive on the support of one antihypertensive medication.
