VIDEO OF PCNL IN SOFT MATRIX STONE

Soft matrix stone appear as fluffy tissues on nephroscopy. They can be teased and removed with the forceps. Many a times Holmium LASER or pneumatic lithotripsy devices are not needed. The matrix stones are composed of mucoptrotein and mucopolysachharides with very less crystalline component.Generally the matrix formation occurs because of infection..mostly by urea splitting proteus vulgaris. They are characteristically radiolucent on IVP and CT scan also reveals soft tissue densities. These stones are best tackled with the PCNL as ESWL is bound to fail. We have a policy to start antibiotics prior to taking up of such infected stones atleast one week prior and continue it for 1 month after the surgery to eradicate the infection and render sterile culture.

Here is a video of PCNL done in matrix stone....
TYPICALLY THEY HAVE A RADIOLUCENT APPEARANCE ON XRAY/IVP
