A 60 y lady presented with severe abdominal distension due to polycystic kidney disease causing huge enlargement (35 cm) of both kidneys. She is on maintenance hemodialysis as she is suffering from ESRD due to ADPKD. The enlarged kidneys were causing mechanical difficulty and distress. After optimising her medical condition preoperatively, she was taken up for simultaneous bilateral open nephrectomy.The procedure was done through a rooftop incision and was uneventful.
