Semen banking : Important thing to be done prior to testiclar cancer management.



Testicular carcinoma affects young people between the age group of 20-40 years.Many of these patients are either recently married or yet to get married.

The implications of treatment of testicular carcinoma on fertility are high.As the gonads are very sensitive to radiotherapy and chemotherapy ; the sperm count can be affected badly after the management of testicular cancer.

It is very essential for the doctor to send the patient for fertility counseling.The sperms should be cryopreserved so the patient can retain his right for fatherhood after he gets cured of testicular cancer.
Cryopreservation requires that a man give 3 samples of semen. Ideally, a semen sample should be collected in a lab after masturbation; if this is not possible, the man may be allowed to collect a sample at home in a sterile wide mouth container or condom without spermicidal agents. 
If possible, collection should be started before surgical removal of the testicle and before chemotherapy or radiation therapy; this allows the greatest number and healthiest sperm to be stored.

As the orchiectomy itself can lead to some decrease in function of contra lateral testicle. 

Before giving a sample, a three day abstinence from the sexual act is required.
